The National Kidney Foundation of Michigan, in partnership with the MDHHS Diabetes Program, in 2012 gathered resources that align with content presented in each Diabetes PATH session. Resources were updated in 2015. The materials are suggested for standardized use by Michigan Diabetes PATH leaders.
